胸腰段脊柱爆裂骨折的不同内固定方式临床疗效对比

摘要：

目的 评价不同内固定方式在胸腰椎脊柱爆裂骨折中的应用效果。方法 选择在我院行前路内固定手术治疗的 26例胸腰段脊柱爆裂骨折患者作为观察组，以同期经后路内固定手术治疗的 24例胸腰段脊柱爆裂骨折患者作为对照组。比较两组手术一般指标、占位矫正率、坐立及行走时间、脊椎矫正效果、切口愈合率及固定准确率。结果 两组的手术时间比较，差异显著（P＜0.05）；观察组的术中出血量及术后引流量均高于对照组（P<0.05）；观察组的占位矫正率明显高于对照组，坐立及行走时间明显早于对照（P<0.05）。观察组的脊柱侧弯、后凸畸形角、椎间高度矫正不佳等情况均优于对照组（P<0.05）；观察组的切口愈合率和固定准确率稍优于对照组（P>0.05）。结论 前路内固定较后路内固定更具优势，临床应用效果良好，是胸腰段脊柱爆裂骨折的理想内固定方式。

Clinical efficacy comparison of different internal fixation methods
on thoracolumbar burst fractures

ABSTRACT: Objective To evaluate the effects of different internal fixation methods on thoracolumbar burst fractures.Methods A total of 26 cases of thoracolumbarspine burst fracture streated with anteriorfixed surgery in our hospital were selected as the observation group. In the same period, 24 cases of thoracolumbar spine burst fractures treated with posterior fixation were selected as the control group.The general indexes, occupancy correction rate,sitting and walking time,spinal correction,incision healing and fixed accuracy were compared between the two groups. Results There was significant differencein operative time between the two groups (P＜0.05).The intraoperative blood loss and postoperative drainage in the observation group were higher than those in the control group (P<0.05).The occupancy correction rate in the observation group was significantly higher than that in the control group,and the sitting time and walking time were significantly earlier than those in the control group (P<0.05).The scoliosis,kyphosisangle and poor intervert ebralh eight correction of the ob-servation group were better than those of the controlgroup (P<0.05).The healing rate and fixed accuracy of the observation group were slightly better than those of the controlgroup (P>0.05).Conclusion Compared with posterior internal fixation,anterior internal fixation has more advantages, it has good clinical application effect,which is the ideal method of thora-columbar burst fracture internal fixation.
