血浆 D-二聚体水平在膝关节置换术后并发深静脉血栓 早期诊断中的意义

摘要：

目的 探讨血浆 D-二聚体水平在膝关节置换术后并发深静脉血栓（DVT）早期诊断中的意义。方法 选取我院收治的 77例膝关节置换术患者作为膝关节置换组，分别于术前及术后第 1、3、7天进行血浆 D-二聚体水平检测，并根据彩色多普勒超声检查结果将其分为 DVT 组（n=24）和非 DVT 组（n=53）。选取同期体检的健康者 70例作为对照组。比较各组间的 D-二聚体水平。结果 术后 1、3、7 d，两组膝关节置换术后患者的的血浆 D-二聚体水平明显高于术前 1 d，且 DVT 组明显高于非 DVT组（P＜0.05）；DVT 组患者术后 1、3、7 d的 D-二聚体水平明显高于对照组（P＜0.05）；非 DVT 组患者术后 1、3、7 d的 D-二聚体水平明显高于对照组（P＜0.05）。结论 血浆 D-二聚体水平检测对膝关节置换术后并发深静脉血栓的早期诊断具有重要的临床意义。

ABSTRACT:Objective To investigate the significance of plasma D-dimer level in early diagnosis of deep venous thrombosis (DVT) after knee arthroplasty. Methods A total of 77 patient sunder going knee arthroplasty surgery in ourhospital were selected as the kneearthroplasty group,and all the patients received plasma D-dimerdetection before operation and1,3 and 7 daysafteroperation.They were divided into DVT group (n=24) and non-DVT group (n=53) according to the results of color Doppler ultrasound. Other 70 physical examinersin the same period were enrolled asthe controlgroup.The plasma D dimerlevelswere compared among the groups. Results At 1,3 and 7 daysafteroperation,the plasma D-dimerlevels of patient sunder going knee arthroplasty surgeryin the two groups were higher than those1 day before operation,and those of the DVT group were higher than thenon-DVT group (P＜0.05).At1,3 and 7 daysafteroperation,the plasma D dimerlevels of the DVT group werehigherthan thoseofthecontrolgroup (P＜0.05).At 1,3 and 7 days after operation,the plasma D-dimer level of the non-DVT group was higher than that of the control group (P＜0.05).Conclusion Plasma D-dimerlevelin earlydiagnosisofDVT after knee arthroplasty hasvital clinical significance                                                                                                                                                                                                                                                                                                                                                                              KEYWORDS: knee arthroplasty;D-dimer;deep vein thrombosis
