凝血功能检测在肝硬化患者肝功能评价中的应用价值

摘要：

目的 检测乙型肝炎肝硬化患者的凝血功能，研究其与肝硬化肝损害的相关性，从而对临床的治疗和预后提供科学指导。方法 选择我院 2014年 3月至 2016年 3月收治的 68例乙型肝炎肝硬化患者作为观察组，另选取 60例健康志愿者作为对照组，分别检测两组研究对象的凝血酶原时间（PT）、凝血酶时间（TT）、活化部分凝血酶原时间（APTT）、纤维蛋白原（FIB），比较检测结果。结果 观察组患者的血浆 PT、TT、APTT均长于对照组，FIB 低于对照组（P＜0.05）。随着肝功能 Child-Pugh分级的增加，肝硬化患者的 PT、TT 升高，FIB 降低，组间比较，差异均有统计学意义（P<0.05）。结论 乙型肝炎肝硬化患者具有明显的凝血功能障碍,容易发生广泛性出血等并发症，监测其变化有助于治疗方案的调整，对判断预后有重要的参考价值。

Application value of blood coagulation detection in the evaluation of liver function
in cirrhotic patients

ABSTRACT: Objective To detectthe coagulation function of patients with hepatitis B cirrhosis and study its correlation with liver cirrhosis,so asto provide scientific guidance forclinical treatment and prognosis. Methods Sixty-eight patients with hepatitis B cirrhosis admitted in our hospital from March 2014 to March 2016 were selected as the observation group,and 60 healthy volunteers were selected as the control group.The prothrombin time (PT),thromb in time (TT), activated partial prothrombin time(APTT) and fibrinogen (FIB)were detected,and there sults of the test were compared. Results The plasma PT,TT and APTT in the observation group were longer than those in the control group,and the FIB level was lower than that in the control group (P＜0.05).With the increase of Child-Pugh classification of liver function,the PT,TT increased,and FIB decreased in patients with cirrhosis,the differences between the two groups were statistically significant(P＜0.05).Conclusion Hepatitis B cirrhosis patients have obvious coagulation dysfunction,prone tooccurthe complications such as extensive bleeding. Monitoring the coagulation function changes will help to adjust the treatment plan,and have important reference value for  judgments of prognosis.
