PHILOS内固定治疗老年肱骨近端骨折临床观察

摘要：

目的 观察PHILOS内固定与LPHP内固定治疗老年肱骨近端骨折的临床效果。方法 收集我院收治的老年肱骨近端骨折患者74例，将所有患者随机分为对照组和观察组，每组37例。对照组采用LPHP内固定治疗，观察组采用PHILOS内固定治疗。比较两组患者的手术时间、术中出血量、愈合时间，肩关节功能恢复情况及治疗过程中的并发症发生情况。结果 观察组的两种骨折类型的手术时间、术中出血量及愈合时间均显著优少于对照组（P<0.05）；肱骨近端三部分骨折的手术时间、术中出血量显著优于肱骨近端四部分骨折（P<0.05），而两种骨折类型的愈合时间比较，差异无统计学意义（P>0.05）。观察组患者的Neer评分优良率为89.19%，明显高于对照组的78.38%（P<0.05）。观察组患者的并发症总发生率为5.41%，显著低于对照组的18.92%（P<0.05）。结论 采用PHILOS内固定治疗老年肱骨近端骨折，临床效果显著，肩关节功能恢复良好，术后并发症发生率低，值得临床推广应用。

Clinic observation of proximal humerus fractures treated with PHILOS internal fixation

ABSTRACT: Objective To observe the clinical effect of LPHP and PHILOS in the treatment of proximal humerus fracture in elderly patients. Methods Seventy cases of proximal humeral fractures in elderly patients were selected and randomly divided into control group and observation group, with 37 cases in each group. The control group used LPHP and the observation group received PHILOS. The operation time, intraoperative blood loss, healing time, shoulder joint function recovery and complications were observed in the two groups. Results The operation time, intraoperative blood loss, healing time of the two types of the observation group were better than those of the control group (P<0.05), the operation time, intraoperative blood loss of three parts of proximal humerus fracture were better than those of the four parts proximal humerus fracture (P<0.05), there was no significant difference in the healing time of the two types (P>0.05). The excellent rate of Neer score in the observation group was 89.19%, which was higher than 78.38% of the control group (P<0.05). The overall incidence of complications in the observation group was 5.41%, which was lower than 18.92% of the control group (P<0.05). Conclusion The clinical effect of PHILOS in the treatment of proximal humerus fracture in elderly patients was remarkable, which has good shoulder joint function recovery, low postoperative complications. It is worthy of clinical application.
