非气管内插管局部麻醉在胸腔镜交感神经切断术中的 应用效果

摘要：

目的 探讨非气管内插管局部麻醉在胸腔镜交感神经切断术中的应用效果。方法 选取 2015年 8月至 2017年 8月我院210例行胸腔镜交感神经切断术的手汗症患者为研究对象，采用随机数字表法将其分为对照组和研究组，每组 105例。对照组患者行气管内插管全身麻醉，研究组患者行非气管内插管局部麻醉，比较两组患者的手术情况、临床效果、术后住院时间及治疗费用。结果 两组患者术中出血量、代偿性多汗发生率、术后住院时间比较，差异不显著（P>0.05）。与对照组比较，研究组患者麻醉准备时间、手术时间、术后清醒时间、总操作时间、下床活动时间、术后咽喉不适发生率、术后 6 h VAS评分、治疗费用明显更优（P<0.05）。结论 非气管内插管局部麻醉可有效避免气管插管对机体的损伤，麻醉效果好，且术后恢复快、花费少，疗效安全可靠，值得在胸腔镜交感神经切断术中进行推广应用。

ABSTRACT: Objective To explore the application effect of nontracheal intubation local an aesthesia in thoracoscopic sympathectomy. Methods From August2015 to August 2017,210 hyperhidrosis patients who underwent thoracoscopic sympathectomy in our hospital were selected as the research objects, and divided into control group and study group according to random numbertable method,with 105 cases in each group.The controlgroup underwent general anesthesia with endo-tracheal intubation,and the study group underwent local anesthesia with non tracheal intubation.The operation situation,clinical effect, postoperative hospital stays and treatment costs were compared between the two groups. Results There were no significant differences between the two groups in the amount of blood loss,incidence of compensatory hyperhidrosis and postoperative hospital stay (P>0.05). Compared with the control group,the anesthesia preparation time,operation time, postoperative awake time,total operation time,activity time out of bed,incidence of postoperative throat discomfort, VASscoreat6 h after operation and treatment cost were significantly better (P<0.05).Conclusion Non tracheal intubation local anaesthesia can effectively avoid the body damage from trachealintubation,with good anesthesia effect,and its has quick recovery and less costs.It is safe and reliable, which worth application in  thoracoscopic sympathectomy.
