闭合性腹部损伤小肠破裂的诊治体会

摘要：

目的 总结闭合性腹部损伤小肠破裂的诊治体会。方法 选取2011 年1 月到2015 年1 月我院收治的70 例闭合性腹部 损伤小肠破裂患者，采用回顾性分析方法探讨闭合性腹部损伤小肠破裂的诊治经验与体会。结果 经检查，患者的破裂部位： 空肠41 例，回肠27 例，十二指肠2 例；治疗后，57 例患者痊愈，12 例患者有效，1 例患者无效，整体治疗有效率为98.6%（69/ 70）；出现术后感染现象的患者有3 例，发生率为4.3%（3/70）；出血1 例，发生率为1.4%（1/70）。结论 在闭合性腹部损伤小肠 破裂的诊治中应当遵循“先救命，再治疗”的理念，应尽量做到早发现、早诊断、早治疗。同时，医护人员还应做好术后并发症 的预防控制与护理干预工作，从而降低术后并发症发生率，提高综合治疗效果。

Experience in diagnosis and treatment of small intestine rupture in closed abdominal trauma

ABSTRACT: Objective To summarize the diagnosis and treatment experience and realization of small intestinal rupture in closed abdominal trauma. Methods From January 2011 to January 2015, 70 patients of small intestinal rupture in closed abdominal trauma were enrolled. A retrospective analysis was conducted to explore the diagnosis and treatment experience and realization of small intestinal rupture in closed abdominal trauma. Results After examination, there were 41 cases of jejunum, 27 cases of ileum and 2 cases of duodenal; after treatment, 57 patients were cured, 12 patients were effective, 1 patient was ineffective, and the overall treatment effective rate was 98.6% (69/70); 3 cases of postoperative infection, the incidence was 4.3% (3/70); 1 case of hemomhage, the incidence was 1.4% (1/70). Conclusion In the treatment of small intestinal rupture in closed abdominal trauma, the concept of "first life-saving, re-treatment" should be followed, and try to early detection, early diagnosis and early treatment. At the same time, medical staff should also do a good job in postoperative prevention and control of complications and nursing intervention, thus reducing the incidence of postoperative complications and improving the comprehensive treatment effect.
