单孔腹腔镜在年轻女性妇科手术中的应用

摘要：

目的 探讨分析单孔腹腔镜在年轻女性妇科手术中的应用效果。方法 将我院在2015 年2 月至2016 年10 月收治的72 例行腹腔镜（腹腔镜下卵巢肿瘤剥除术或输卵管切除术、输卵管开窗术、盆腔粘连松解术或输卵管造口术）的年轻女性患者 的临床资料纳入研究，根据手术方式的不同将患者分为观察组（36 例）与对照组（36 例）。对照组患者采用传统腹腔镜手术治 疗，观察组患者采用单孔腹腔镜治疗，对比分析两组患者的住院时间、术后排气时间、术中出血量、术后疼痛评分、手术时间、术后切口愈合及恢复情况。结果 观察组患者术中出血量、手术时间与对照组比较，无明显差异（P>0.05），然而其术后排气时 间、术后疼痛评分及住院时间均明显低于对照组，差异有统计学意义（P<0.05）。两组患者切口均为甲级愈合且无脐疝发生， 术后均未出现并发症。结论 相比于传统的腹腔镜手术，单孔腹腔镜手术未明显增加手术时间及术中出血量，但其在减轻患 者术后疼痛，以及缩短住院时间，减少术后瘢痕方面具有明显功效，值得临床推广。

Application of single-hole laparoscopy in gynecological surgery of young women

ABSTRACT: Objective To explore the application effect of single -hole laparoscopy in gynecological surgery of young women. Methods Seventy-two patients who underwent laparoscopic (laparoscopic ovarian tumor stripping or salpingectomy, fallopian tube fenestration, pelvic adhesions or oviduct ostomy) in our hospital during the period from February 2015 to October 2016 were selected as the study objects, and divided into control group and observation group according to different surgical methods, with 36 cases in each group. The patients in the control group were treated with traditional laparoscopic surgery, while the patients in the observation group were treated with single-hole laparoscopy. The time of hospitalization, postoperative exhaust time, amount of intraoperative blood loss, postoperative pain scores, operation time, incision healing results and recovery situation between the two groups were compared. Results There were no significant differences in the amount of intraoperative blood loss and operation time between the two groups (P>0.05). However, the postoperative exhaust time, postoperative pain score and hospital stay in the observation group were significantly lower than those in the control group (P<0.05). The incision of the two groups was Grade A healed, without umbilical hernia and complication occurred. Conclusion Compared with traditional laparoscopic surgery, single-hole laparoscopic surgery won``t significantly increase the operation time and intraoperative blood loss, but it has obvious effect on reducing postoperative pain, shortening the patients`` hospital stay and reducing postoperative scarring, which is worthy of clinical promotion.
