左布比卡因与布比卡因腰麻用于剖宫产术的效果比较

摘要：

目的 观察左布比卡因与布比卡因腰麻用于剖宫产术的麻醉效果。方法 选择我院2015年7月至2016年10月行剖宫产术的孕妇120例，按照随机数字表法分为A 组和B 组，各60 例。A组和B组分别在蛛网膜下腔内注入0.5%左布比卡因和0.5%布比卡因各2.2 mL。比较两组产妇感觉、运动阻滞起效时间及恢复时间，术中产妇血压、心率及新生儿Apgar 评分情况。 结果 两组产妇最高感觉阻滞平面时间、运动阻滞起效时间、感觉阻滞恢复时间及两组新生儿Apgar 评分比较，差异无统计学 意义（P>0.05）；A 组运动阻滞恢复时间明显短于B 组（P<0.05）；B 组产妇给药后5、10 min 的SBP、DBP、HR 均明显低于A 组（P<0.05）。结论 左布比卡因腰麻用于剖宫术效果确切，术后运动功能恢复快，值得临床推广。

Effect comparison of levobupivacaine and bupivacaine for lumbar anesthesia in cesarean section

ABSTRACT: Objective To observe the anesthetic effect of levobupivacaine and bupivacaine for lumbar anesthesia in cesarean section. Methods From July 2015 to October 2016, 120 cases of pregnants with cesarean section admitted in our hospital were selected and divided into group A and group B according to the random number table method, with 60 cases in each group. The group A was injected with 2.2 mL 0.5% levobupivacaine into the subarachnoid space, while the group B was injected with 2.2 mL 0.5% bupivacaine into the subarachnoid space. The onset and recovery time of sensory nerve and motor nerve block, the maternal blood pressure and heart rate during operation, the Apgar score of neonate were recorded and compared between the two groups. Results There were no significant differences in the onset time of sensory nerve and motor nerve block, the recovery time of sensory nerve and Apgar score between the two groups (P>0.05). The recovery time of motor nerve block in the group A was significantly shorter than that in the group B (P<0.05). The maternal SBP, DBP, HR at 5 and 10 min after administration in the group B were significantly lower than those in the group A (P<0.05). Conclusion Levobupivacaine for lumbar anesthesia in cesarean section has good effect, and can be conducive to the postoperative recovery of motor function, which is worthy of clinical application.
