腹腔镜下子宫肌瘤剔除术与开腹手术的临床疗效对比

摘要：

目的 比较腹腔镜下子宫肌瘤剔除术与开腹手术的临床疗效。方法 回顾性分析2015年3月至2017年3月我院妇科收治的70例子宫肌瘤患者临床资料，将接受腹腔镜剔除术的35 例患者设为观察组，接受传统开腹手术的35 例设为对照组。 记录两组患者的手术切口、排气时间、术中出血量等手术指标，并比较患者术后并发症发生情况、切口愈合时间及住院时间。 结果 观察组患者的切口长度、排气时间、术中出血量均优于对照组，差异有统计学意义（P<0.05）。观察组并发症发生率为 2.86%，切口愈合时间为（14.38±2.14）d，住院时间为（5.12±1.08）d，均显著优于对照组的17.14%、（18.25±3.17）d、（8.23±1.34）d，差 异有统计学意义（P<0.05）。结论 开腹手术和腹腔镜剔除术均可有效剔除子宫肌瘤，但腹腔镜手术创伤小，患者术后的恢复 更快，且可明显降低并发症发生率，安全性更高，值得推广。

Clinical efficacy comparison of laparoscopic myomectomy and laparotomy

ABSTRACT: Objective To compare the clinical efficacy of laparoscopic myomectomy and laparotomy. Methods The clinical data of 70 cases of patients with uterine fibroids treated in our hospital from March 2015 to March 2017 were retrospectively analyzed, the 35 cases undergoing laparoscopic myomectomy were enrolled as the observation group, the other 35 cases undergoing traditional open surgery were enrolled as the control group. The operation incision, exhaust time and the amount of bleeding during operation were recorded in the two groups, and the postoperative complications, the time of incision healing and hospital stay were compared between the two groups. Results The incision length, exhaust time and intraoperative blood loss in the observation group were significantly better than those in the control group (P<0.05). In the observation group, the incidence rate of complications was 2.86%, the incision healing time was (14.38±
2.14) d and the hospitalization duration was (5.12±1.08) d, which were significantly better than 17.14%, (18.25±3.17) d, (8.23±1.34) d in the control group, the differences were statistically significant (P<0.05). Conclusion Laparotomy and laparoscopic myomectomy both can effectively remove uterine fibroids, but laparoscopic myomectomy has advantages of small trauma, rapid postoperative recovery, and it can significantly reduce the incidence of complications, with high security, which is worthy of promotion.
