DHS与PFNA治疗老年股骨粗隆间骨折的临床效果比较

摘要：

目的 分析动力髋螺钉固定（DHS）与防旋股骨髓内钉固定（PFNA）治疗老年股骨粗隆间骨折患者的临床效果及体会。 方法 回顾性分析我院收治的67 例股骨粗隆间骨折患者的临床资料，将其按治疗方式分为观察组（34 例）与对照组（33 例）， 对照组患者采用DHS 进行治疗，观察组患者采用PFNA 进行治疗，比较两组患者的各手术指标、髋关节功能恢复情况。结果 观察组患者的手术时间、术中失血量、手术切口长度、负重时间均显著优于对照组（P<0.05）。观察组患者的髋关节功能恢复 优良率为76.47%，显著高于对照组的51.52%（P<0.05）。结论 老年股骨粗隆间骨折患者应用PFNA 治疗的效果优于DHS，其 可促进患者髋关节功能的恢复，具有较高的临床应用价值。

Effect comparison of DHS and PFNA on femoral intertrochanteric fracture in elderly patients

ABSTRACT: Objective To analyze the therapy effect and experiences of dynamic hip screw fixation (DHS) and proximal femoral nail anti-rotation (PFNA) on femoral intertrochanteric fracture in elderly patients. Methods The clinical data of 67 cases of patients with femoral intertrochanteric fracture admitted in our hospital were retrospectively analyzed, they were divided into observation group (34 cases) and control group (33 cases) according to the treatment ways. Patients in the control group were treated with DHS, while patients in the observation group were treated with PFNA. The surgical indicators, hip function recovery in the two groups were compared. Results The operation time, intraoperative blood loss, length of incision and weight-bearing time in the observation group were significantly better than those of the control group (P<0.05). The excellent and good rate of hip joint function recovery in the observation group was 76.47%, which was significantly higher than 51.52% of the control group (P<0.05). Conclusion In elderly patients with femoral intertrochanteric fracture, PFNA treatment has better effect than DHS, which can promote the recovery of patients`` hip function, it has high clinical value.
