腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石的临床应用效果

摘要：

目的 评价腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石的临床疗效。方法 选取我院2016年2月至2017年2月收治的76例胆囊结石合并胆总管结石患者作为研究对象，按随机盲选法分为对照组和观察组，各38 例。对照组给予传统开腹手术，观察组给予腹腔镜联合胆道镜治疗，对比两组临床治疗效果。结果 两组手术时间比较，无明显差异（P>0.05）；观察组胃 肠功能恢复时间和住院时间均明显短于对照组（P<0.05）；观察组术中出血量和术中补液量均显著少于对照组（P<0.05）。观 察组并发症总发生率为7.89%，明显低于对照组的28.95%，差异有统计学意义（P<0.05）。结论 腹腔镜联合胆道镜治疗胆囊 结石合并胆总管结石临床效果显著，能够控制术中出血量及术中补液量，促进患者胃肠功能恢复，并降低并发症发生率，改 善手术预后效果，值得临床推广应用。

Clinical application effect of laparoscope combined with choledochoscope in the treatment of cholecystolithiasis complicated with choledocholithiasis

ABSTRACT: Objective To evaluate the clinical effect of laparoscope combined with choledochoscope in the treatment of cholecystolithiasis complicated with choledocholithiasis. Methods From February 2016 to February 2017, 76 cholecystolithia-sis complicated with choledocholithiasis patients treated in our hospital were selected as the research objects and divided into control group and observation group according to the random blind method, with 38 cases in each group. The control group was treated with traditional open operation, the observation group was given laparoscope combined with choledochoscope therapy. The clinical therapeutic effects of the two groups were compared. Results There was no significant difference in the operation time between the two groups (P>0.05); the gastrointestinal function recovery time and length of stay in the observation group were significantly shorter than those of the control group (P<0.05); the amount of intraoperative blood loss, intraoperative rehydration in the observation group were significantly less than those of the control group (P<0.05).The total incidence of complications in the observation group was 7.89%, which was significantly lower than 28.95% in the control group, the difference was statistically significant (P<0.05). Conclusion Laparoscope combined with choledochoscope in the treatment of cholecystolithiasis complicated with choledocholithiasis has remarkable curative effect, which can control the amount of intraoperative blood loss and rehydration during, promote gastrointestinal functional recovery, and reduce the incidence of complications, it is worth clinical promotion and application.
