彩色多普勒超声结合四维超声诊断胎儿脐带绕颈的临床应用价值

摘要：

目的 探讨应用彩色多普勒超声结合四维超声在胎儿脐带绕颈中的诊断价值。方法 随机选取2015年6月1日至2016年6月1日在我院住院分娩的产妇300例为研究对象，采用随机数字表法将其分为对照组和观察组，每组150例。对照组产妇采用彩色多普勒超声进行产检，观察组产妇应用彩色多普勒超声结合四维超声进行产检，比较两组胎儿脐带绕颈诊断的准确率。结果 观察组在绕颈1周、2周、3周及3周以上的准确率明显高于对照组（P<0.05）；两组产妇共出现胎儿脐带绕 颈98 例，其中胎儿宫内窘迫和新生儿窒息的发生率分别为15.31%、12.24%；未发生胎儿脐带绕颈产妇202例，其中胎儿宫内窘迫和新生儿窒息发生率分别为2.97%、1.98%，两组差异有统计学意义（P<0.05）。结论 应用彩色多普勒超声结合四维超 声对产妇进行产检，能够提高胎儿脐带绕颈诊断的准确率，有较高的可靠性和安全性，可为增强脐带绕颈胎儿围生期护理及救治提供依据，提高产妇及新生儿的生存质量。

Clinical value of color doppler ultrasound combined with four-dimensional ultrasound in diagnosis of umbilical cord around the neck

ABSTRACT: Objective To discuss the clinical value of color d oppler ultrasound combined with four -dimensional ultrasound in diagnosis of umbilical cord around the neck. Methods Three hundred cases of hospitalized childbirth puerpera in our hospital from June 1, 2015 to June 1, 2016 were randomly selected as the research objects, all the patients were divided into control group and observation group with random number table method, with 150 cases in each group. The control group used color doppler ultrasound for prenatal, while the observation group accepted color doppler ultrasound combined with four dimensions ultrasound for prenatal. The diagnostic accuracy of fetal umbilical cord around the neck were compared between the two groups. Results After checked, in umbilical cord around the neck 1 laps, 2 laps, 3 laps and more, the accuracy rates of the observation group were significantly higher than those of the control group (P<0.05). A total of 98 cases in umbilical cord around the neck, it showed the incidence rates of fetal distress and neonatal asphyxia were respectively 15.31% and 12.24%，two kinds of incidence rate in 202 cases of puerperas without umbilical cord around the neck were respectively 2.97% and 1.98%, the differences were statistically significant （P<0.05）. Conclusion Color doppler ultrasound combined with four-dimensional ultrasound applied in diagnosis for prenatal can improve the diagnostic accuracy rate of fetal umbilical cord around the neck, with high reliability and security, it can provide evidence for nursing and treatment of perinatal period of umbilical cord around the neck, and improves the survival quality of maternal and neonatal.
