功能性鼻窦镜手术联合地塞米松鼻腔冲洗治疗鼻窦炎疗效及安全性研究

摘要：

目的 探讨功能性鼻窦镜手术联合地塞米松鼻腔冲洗治疗鼻窦炎疗效及安全性。方法 采用回顾性研究方法，选择2013年1月至2016年1月在我院诊治的鼻窦炎患者120例，根据治疗方法的不同分为观察组与对照组，各60例。所有患者都在鼻内镜下行Messerklinger术式。对照组术后冲洗液为0.9%氯化钠溶液，观察组冲洗液为0.9%氯化钠溶液200 mL混合10 mg地塞米松+16 万U庆大霉素+α-糜蛋白酶8 000 U，治疗2 个月后，观察两组的临床效果。结果 治疗后观察组与对照组的总 有效率分别为96.67%和85.00%，观察组的总有效率明显高于对照组（P<0.05）；观察组治疗后的鼻内镜评级1级者多于对照 组（P<0.05），2 级比较无明显差异（P>0.05），3、4、5 级者均明显少于对照组（P<0.05）。结论 功能性鼻窦镜手术联合地塞米松鼻腔冲洗治疗鼻窦炎具有很好的疗效与安全性，能促进疾病的康复，值得在临床上推广应用。

Study on the efficacy and safety of functional endoscopic sinus surgery combined with dexamethasone nasal rinse in the treatment of patients with nasosinusitis

ABSTRACT: Objective To investigate the clinical efficacy and safety of functional endoscopic sinus surgery combined with dexamethasone nasal rinse on patients with nasosinusitis. Methods A retrospective study was conducted in 120 cases of patients with nasosinusitis in our hospital from January 2013 to January 2016, all the patients were divided into control group (n =60) and observation group (n =60) according to the type of treatment. All patients were treated with nasal endoscopic Messerklinger surgery, patients in the control group were given 0.9% sodium chloride solution, and patients in the observation group were given 0.9% sodium chloride solution (200 mL), dexamethasone (10 mg), gentamicin (160 000 U) and α-chymotrypsin (8 000 U). The clinic effect was observed in the two groups after treated for 2 months. Results After the treatment, the total effective rate of the observation group and the control were 96.67%, 85.00% respectively, and the total effective rate of the observation group were significantly higher than that of the control group (P<0.05). After the treatment, the patients of nasal endoscopy rating level 1 of the observation group were more than that of the control group, and level 3, 4, 5 were less than those of the control group (P<0.05), there was no difference in level 2 between the two groups (P>0.05). Conclusion Functional endoscopic sinus surgery combined with dexamethasone nasal irrigation has remarkable clinical effects and safety in the treatment of patients with nasosinusitis, which can promote the rehabilitation of the disease, so it is worth popularizing and applying in clinic.
