52例1型糖尿病患者的临床特征与心电图分析

摘要:

目的 观察1 型糖尿病(T1DM)患者心电图异常改变及其特点。方法 收集52例T1DM住院住院患者临床资料,分析心电图报告,观察和记录患者年龄、病程、性别、血压、血脂、尿β2微球蛋白(尿β2-MG)等对心电图的影响。结果 52例患者中38例心电图检测出现异常,发生率占73.08%。心电图变化以ST-T缺血改变最常见,占36.84%,其次为窦性心动过速、窦性心动过缓、T波低平、倒置或双向、Q-T间歇延长等。心电图异常发生率与糖尿病的病程长短有关,合并高血压、高血脂、高尿β2-MG者心电图异常发生率明显高于无合并者,差异有统计学意义(P<0.05)。结论 T1DM患者心电图异常发生率高,与患者性别、年龄无相关性,患者病程以及血压、血脂、尿β2-MG控制水平等因素是T1DM心血管异常的发病基础。

    Analysis of clinical features and electrocardiogram in 52 patients with type 1 diabetes mellitus

    ABSTRACT: Objective To observe the electrocardiogram (ECG) abnormal changes and characteristics of patients with type 1 diabetes (T1DM). Methods The clinical data of 52 T1DM patients in our hospital were collected, and the ECG report were analyzed, the effects of age, course of disease, gender, blood pressure, blood lipid, urine β2 microglobulin (urine β2-MG) were observed and recorded. Results Of 52 cases of patients, there were 38 cases with abnormal ECG, the incidence rate was 73.08%. Electrocardiogram change in ST-T ischemia is most common, accounting for 36.84%, followed by sinus tachycardia, sinus bradycardia, T wave flat, inverted or bidirectional, prolongation of the Q-T interval etc. The incidence of abnormal ECG was related to the duration of diabetes. The incidence rates of abnormal ECG in patients with hypertension, high blood lipid and high urine β2-MGwere significantly higher than those in the patients without the above diseases (P<0.05). Conclusion he incidence rate of abnormal ECG in patients with T1DM was high, which is not correlated with gender and age, the course of disease, blood pressure, blood lipid and urine β2-MGlevel are the basis of T1DM cardiovascular abnormalities.
