高频超声对不同关节部位活跃期类风湿关节炎辅助诊断效果的对比研究

摘要：

目的 分析高频超声对不同关节部位活跃期类风湿关节炎辅助诊断效果。方法 选择本院新确诊的类风湿关节炎患者104例，比较分析高频超声对不同关节部位活跃期类风湿关节炎辅助诊断的效力。结果 与金标准相比，高频超声方法诊断近端指间关节类风湿活跃期患者的灵敏度为86.20%，漏诊率为13.80%，误诊率为19.96%；诊断掌指关节类风湿活跃期患者的灵 敏度为82.50%，漏诊率为17.50%，误诊率为18.03%；诊断腕关节类风湿活跃期患者的灵敏度为72.73%，漏诊率为27.27%，误诊率为25.26%；诊断膝关节类风湿活跃期患者的灵敏度为91.49%，漏诊率为8.51%，误诊率为9.72% 。结论 高频超声检查手段在活跃期的类风湿关节炎患者的诊断中，对膝关节的诊断效率最高，对腕关节的诊断效率稍差。总体来说值得临床推荐。

Comparison the effect of high-frequency ultrasonography in the auxiliary diagnosis of active

rheumatoid arthritis of different joints

ABSTRACT: Objective To investigate the effect of high-frequency ultrasonography in the auxiliary diagnosis of active rheumatoid arthritis of different joints. Methods One hundred and four patients newly diagnosed with rheumatoid arthritis were selected, and the diagnostic efficacy of high-frequency ultrasound on active rheumatoid arthritis of different joints were compared and analyzed. Results Compared with the gold standard, the sensitivity of the high frequency ultrasound in diagnosing patients with proximal interphalangeal rheumatoid arthritis in active phase was 86.20%, the missed diagnosis rate was 13.80%, and the misdiagnosis rate was 19.96%. The sensitivity in diagnosing patients with metacarpophalangeal rheumatoid in active phase was 82.50%, the missed diagnosis rate was 17.50%, and the misdiagnosis rate was 18.03%. The sensitivity in diagnosing patients with wrist rheumatoid in active phase was 72.73%, the missed diagnosis rate was 27.27%, and the misdiagnosis rate was 25.26%. The sensitivity in diagnosing patients with knee rheumatoid in active phase was 91.49%, the missed diagnosis rate was 8.51%, and the misdiagnosis rate was 9.72%. Conclusion High-frequency ultrasonography is the most effective for the diagnosis of knee joint in patients with rheumatoid arthritis, and the diagnostic efficiency of the wrist is slightly worse. Which is overall worthy of clinical recommendation.
