负压封闭引流技术在高位肛周脓肿中的应用效果分析

摘要：

目的 观察负压封闭引流技术治疗高位肛周脓肿的临床效果。方法 将我院收治的68 例高位肛周脓肿患者随机分为对 照组和观察组，各34 例，分别采用传统切开挂线术与负压封闭引流术治疗。比较两组的治疗效果、治疗前后疼痛、肛门失禁 改善情况，比较两组治疗前后血清TNF-琢、EGF 含量。结果 观察组治疗后4 周，总有效率显著高于对照组（P<0.05）。治疗后，两组VAS 及Wexner 评分、血清TNF-α、EGF 含量均改善（P<0.05）。观察组VAS、Wexner 评分显著低于对照组，TNF-α 含量显 著低于对照组，EGF 含量高于对照组（P<0.05）。结论 采用负压封闭引流治疗高位肛周脓肿，临床效果显著，可显著缓解疼 痛，改善肛门失禁，这可能与抑制TNF-α 水平，上调EGF 水平有关。

Application effect of vacuum sealing drainage in high perianal abscess

ABSTRACT: Objective To observe the clinical effect of negative pressure sealing drainage in the treatment of high perianal abscess. Methods Sixty-eight patients with high perianal abscess admitted in our hoapital were randomly divided into control group and observation group, with 34 cases in each group, the incision -thread -drawing procedure and vacuum sealing drainage were used respectively. The therapeutic effects, pain and anus incontinence were compared between the two groups before and after treatment, and the levels of TNF-α and EGF in the two groups were compared before and after treatment. Results Four weeks after treatment, the total effective rate in the observation group was significantly higher than that in the control group (P<0.05). After treatment, the VAS, Wexner scores, levels of serum TNF-α and EGF significantly improved in both groups (P<0.05). The VAS and Wexner scores in the observation group were significantly lower than those in the control group, the content of TNF-α in the observation group was significantly lower than that in the control group, and the EGF content was higher than that in the control group (P<0.05). Conclusion The use of vacuum sailing drainage in the treatment of high perianal abscess has exact clinical effect, and it can significantly relieve pain, improve anal incontinence, which may be related to inhibition of TNF-α levels, up-regulation of EGF levels.
