产后抑郁对产妇阴道流血量、初乳始动时间及泌乳量的影响

摘要：

目的 探讨产后抑郁对产妇阴道流血量、初乳始动时间及泌乳量的影响，以便完善产妇和新生儿的护理工作。方法 对200 例产妇采用自评抑郁量表测定其心理状况，根据测试结果分为对照组与抑郁组，各100 例。比较两组产后阴道流血量、初乳始 动时间及泌乳量。结果 对照组阴道流血量<200 mL 和>500 mL 的人数明显优于抑郁组（P<0.05）；两组阴道流血量为200～500 mL 产妇人数比较，差异不显著（P>0.05）；对照组初次泌乳时间在产后24、72 h 内的人数明显优于抑郁组（P<0.05）；两组初 次泌乳时间在产后48 h 内的产妇人数比较，差异不显著（P>0.05）；产后各时段抑郁组泌乳量均明显少于对照组，差异具有统计 学意义（P<0.05）。结论 产后抑郁会加大产妇阴道流血量、推迟初乳始动时间、减少泌乳量，对产妇有不良影响。

Effect of postpartum depression on maternal vaginal bleeding, colostrum initiation time and lactation yield

ABSTRACT: Objective To explore the effect of postpartum depression on maternal vaginal bleeding, colostrum initiation time and lactation yield, in order to improve the maternal and neonatal care. Methods The psychological status of 200 puerperant was measured by self-rating depression scale and the puerpera were divided into control group and depression group according to the test results, with 100 cases in each group. The maternal vaginal bleeding, colostrum initiation time and lactation yield were compared between the two groups. Results The numbers of maternal vaginal bleeding < 200 mL and >500 mL in the control group were better than those in the depression group (P<0.05); there was no significant difference in the number of maternal vaginal bleeding between 200 mL and 500 mL in the two groups (P>0.05). The numbers of colostrum initiation time within postpartum 24, 72 h in the control group were better than those in the depression group (P<0.05); there was no significant difference in the number of colostrum initiation time within postpartum 48 h between the two groups (P>0.05). The lactation yield in each period of the control group were shorter than those of the depression group (P<0.05). Conclusion Postpartum depression can increase the maternal vaginal bleeding, delay the colostrum initiation time, and reduce the lactation yield, which has bad effect on the puerperant.
