D1 和D2 淋巴结清扫术治疗胃癌的疗效和安全性对比

摘要：

目的 比较胃癌治疗中D1、D2 淋巴结清扫术的临床疗效及安全性。方法 选取本院2011年1月至2012年3月收治的胃癌患者共60例作为研究对象，依照治疗方式分为对照组和观察组，每组30 例。对照组予以D1 淋巴结清扫术，观察组予以 D2 淋巴结清扫术，比较两组患者的临床疗效（1、3、5 年生存率）及术后并发症发生情况。结果 两组患者的1 年生存率比较，差异不具有统计学意义（P>0.05）；观察组患者的3 年和5 年生存率均明显高于对照组，差异具有统计学意义（P<0.05）。观察 组患者的吻合口瘘、胰瘘、切口感染、肺部感染的发生率均明显低于对照组，差异具有统计学意义（P<0.05）。结论 与D1 淋巴 结清扫术比较，在胃癌根治术中，D2 淋巴结清扫术效果更显著，安全性更高，值得临床推广使用。

Clinical effect and safety comparison between D1 and D2 lymph nodes dissection in the treatment of gastric cancer

ABSTRACT: Objective To compare the clinical effect and safety between the D1, D2 lymph nodes dissection in the treatment of gastric cancer. Methods From January 2011 to March 2012, a total of 60 patients with gastric cancer treated in our hospital were selected as the study objects, and divided into control group and observation group according to the treatment methods, with 30 cases in each group. The control group was given D1 lymph nodes dissection, and the observation group was given D2 lymph nodes dissection. The clinical curative effects (1, 3, 5 years survival rate) and incidence of complications were compared between the two groups. Results There was no significant difference in the 1 year survival rate between the two groups (P>0.05). The 3 years and 5 years survival rate in the observation group were higher than those in the control group, and the differences were statistically significant (P <0.05). The incidences of anastomotic fistula, pancreatic fistula, incision infection and pulmonary infection in the observation group were obviously higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Compared with D1 lymph nodes dissection, D2 lymph nodes dissection has better therapeutic efficacy and higher safety in the treatment of gastric cancer, which is worthy of promotion and application.
