腹腔镜联合胆道镜保胆取石术治疗胆囊结石的临床效果

摘要：

目的 研究并分析腹腔镜联合胆道镜保胆取石术治疗胆囊结石患者的临床效果。方法 将我院收治的128 例胆囊结石 患者按照随机数字表将其分为对照组（64 例）和观察组（64 例）。对照组接受腹腔镜胆囊切除术，观察组接受腹腔镜联合胆道 镜保胆取石术。观察对比两组一期取石率、并发症发生情况、手术时间、术中出血量、肠蠕动恢复时间、饮食恢复时间以及住 院时间。结果 与对照组相比，观察组患者的一期取石率明显更高，胆汁瘘发生率及并发症总发生率明显更低（P<0.05）。与对 照组比较，观察组患者手术时间较长，术中出血量较少（P<0.05）。两组患者的肠蠕动恢复时间、饮食恢复时间及住院时间比 较，无显著差异（P>0.05）。结论 在胆囊结石患者的治疗过程中，腹腔镜联合胆道镜保胆取石术能够显著降低患者的术中出血 量，且不会延长患者术后的康复时间，还能够保留胆囊功能，值得临床推广应用。

Clinical effect of laparoscopic joint choledochoscope in the treatment of gallbladder stone

ABSTRACT: Objective To study and analyze the treatment effect of laparoscopic joint choledochoscope in the treatment of gallbladder stone. Methods A total of 128 cases of gallbladder stone treated in our hospital were divided into control group (64 cases) and observation group (64 cases) according to random number table. The control group received choledochoscope, while the observation group received laparoscopic joint choledochoscope. The primary stone removal rate, complications, operation time, intraoperative blood loss, recovery time of intestinal peristalsis, eating time, hospitalization in the two groups were observed and compared. Results Compared with the control group, the primary stone removal rate in the observation group was higher, and the incidence of bile leakage and total complication rate were lower (P<0.05). Compared
with the control group, the operation time in the observation group was longer, the amount of intraoperative blood loss was fewer (P<0.05). There were no significant in the recovery time of intestinal peristalsis, eating time, hospitalization time in the two groups (P>0.05). Conclusion Laparoscopic joint choledochoscope can obviously reduce the intraoperative blood loss without prolong the postoperative recovery time, and retain gallbladder function in the treatment of gallbladder stone, which is worthy of application.
