Lp-PLA2与颈动脉粥样硬化斑块超声表现对冠脉斑块稳定性的预测价值

摘要:

目的 探讨冠心病患者Lp-PLA2水平及颈动脉粥样硬化斑块超声表现与冠状动脉易损斑块之间的相关性。方法 选取 2014年8月至2016年7月我院收治的冠心病患者101例,术前均行血浆Lp-PLA2检测及颈动脉超声检查,随后行冠状动脉造影术(CAG)及血管内超声(IVUS)检查。根据CAG及IVUS术后结果分为对照组、稳定斑块组和易损斑块组。结果 吸烟史、高血压病是影响患者冠脉易损斑块形成的独立危险因素(P<0.05);稳定斑块组和易损斑块组的血清Lp-PLA2检测水平显著高于对照组(P<0.05);易损斑块组的Lp-PLA2水平高于稳定斑块组(P<0.05);易损斑块组中颈动脉不稳定斑块检出率高于稳定斑块组和对照组(P<0.05);Lp-PLA2水平、颈动脉血管超声表现和冠状动脉易损斑块之间具有正相关性。结论 血浆Lp-PLA2水平检测联合颈动脉血管超声检查可作为冠脉斑块稳定性的预测指标。

    Predictive value of Lp-PLA2 and carotid atherosclerotic plaque ultrasonography on coronary artery plaque stability

    ABSTRACT: Objective To investigate the correlation between Lp -PLA2 levels and carotid atherosclerotic plaque ultrasonography and coronary artery vulnerable plaque in patients with coronary heart disease. Methods One hundred and one patients with coronary heart disease in our hospital from August 2014 to July 2016 were selected, and all the patients received Lp-PLA2 examination and carotid ultrasound examination before operation, the detection of coronary angiography (CAG) and intravascular ultrasound (IVUS) were performed later. According to the results of CAG and IVUS, the patients were divided into control group, stable plaque group and vulnerable plaque group. Results Smoking and hypertension were independent risk factors of coronary artery vulnerable plaque formation (P<0.05). The levels of plasma Lp-PLA2 in the stable plaque group and vulnerable plaque group were significantly higher than that in the control group (P<0.05). The level of Lp-PLA2 in the vulnerable plaque group was higher than that in the stable plaque group and control group (P<0.05). The detection rate of carotid artery unstable plaque in vulnerable plaque group was higher than that in stable plaque group (P<0.05). There was a positive correlation between Lp-PLA2 level, carotid artery ultrasonography and coronary artery vulnerable plaque. Conclusion The detection of plasma Lp -PLA2 levels combined with carotid ultrasonography can be used as a predictive indicator of coronary plaque stability.
