血清和脑脊液中NSE 的变化在小儿中枢神经系统感染中的临床价值分析

摘要:

目的 观察血清、脑脊液中NSE含量变化在小儿中枢神经系统感染中的临床意义。方法 将我院儿科住院部收治的86例中枢神经系统感染患儿作为观察组,按疾病种类分为病脑组(30例)、化脑组(28例)、结脑组(28例),将同期入院的30例疑诊为颅内感染而抽取脑脊液、经临床排除中枢神经系统病变的患儿作为对照组。比较四组治疗前后血清、脑脊液中NSE含量情况。结果 治疗前,病脑组、化脑组、结脑组的血清、脑脊液中NSE含量均显著高于对照组(P<0.05);病脑组和结脑组血 清、脑脊液中NSE含量高于化脑组(P<0.05)。治疗后,病脑组、化脑组、结脑组血清、脑脊液中NSE水平较治疗前显著下降(P<0.05),结脑组患儿脑脊液、血清NSE 及病脑组、化脑组血清NSE与对照组相比,差异仍显著(P<0.05)。结论 血清、脑脊液 中NSE水平可作为中枢神经系统感染患儿脑损伤的标志物之一,并可根据血清、脑脊液中NSE 变化水平的差异,区分三种颅内感染患儿脑损伤程度,提示预后。

    Clinical significance analysis of changes of NSE in serum and cerebrospinal fluid in children with central nervous system infection

    ABSTRACT: Objective To observe clinical significance of NSE contents in serum and cerebrospinal fluid changes in children with central nervous system infection. Methods A total of 86 children with central nervous system infection treated in our hospital were enrolled as observation group, the children further divided into groups of virus meningitis (n=30), suppurative meningitis (n=28) and tubercular meningitis (n=28) according to the disease types, and another 30 cases suspected diagnosis of intracranial infection, extraction of cerebrospinal fluid and exclusion of central nervous sys tem lesions were set as control group. NSE contents in serum and cerebrospinal fluid before and after treatment were observed in the four groups. Results Before treatment, NSE contents in serum and cerebrospinal fluid in the virus meningitis, suppurative meningitis and tubercul or meningitis group were significantly higher than those in the control group (P<0.05). Virus meningitis group and tubercular meningitis group had significantly higher NSE contents in serum and cerebrospinal fluid compared with suppurative meningitis group (P<0.05). After treatment, NSE contents in serum and cerebrospinal fluid in the virus meningitis, suppurative meningitis and tubercul or meningitis group significantly decreased (P<0.05); there were still statistically significant differences in NSE contents in serum and cerebrospinal fluid between tubercular meningitis group and the control group, and in serum between virus meningitis, suppuratine merringitis groups and the control group (P<0.05). Conclusion NSE contents in serum and cerebrospinal fluid can be used as one of the markers of brain damage in children with central nervous system infection, and it can distinguish three types of brain damage according to the changes of NSE in cerebrospinal fluid and serum, which associate with prognosis.
