人工关节置换与内固定治疗老年骨质疏松性不稳定型股骨粗隆间骨折的对比分析

摘要：
目的 分析比较人工关节置换术与内固定术分别治疗老年骨质疏松性不稳定型股骨粗隆间骨折的治疗效果。方法 将我 院2013年6月至2016年5月收治的78例老年骨质疏松性不稳定型股骨粗隆间骨折患者按随机数字表法分为置换组和固定组，每组39例，其中置换组应用人工关节置换术，固定组行内固定术治疗。观察并比较两组患者的治疗效果。结果 两组患者的术中出血量及并发症发生率比较，差异无统计学意义（P>0.05）；置换组的手术时间和术后卧床时间均显著优于固定组，差异有统计学意义（P<0.05）。置换组患者的治疗总有效率为92.31%，固定组患者的治疗总有效率为79.49%，置换组总有效 率明显高于固定组，差异有统计学意义（P<0.05）。结论 人工关节置换术治疗老年骨质疏松性不稳定型股骨粗隆间骨折的临床疗效显著，值得临床应用。

Curative effect comparison of artificial joint replacement and internal fixation in the treatment
of osteoporotic unstable intertrochanteric fractures in senile patients

ABSTRACT: Objective To analyze and compare the curative effect of artificial joint replacement and internal fixation in the treatment of osteoporotic unstable intertrochanteric fractures in senile patients. Methods Seventy-eight cases of senile patients with osteoporotic unstable intertrochanteric fractures in our hospital from June 2013 to May 2016 were divided into the replacement group and fixation group by random number table, with 39 cases in each group. The replacement group was treated with artificial joint replacement, while fixation group was treated with internal fixation. The therapeutic effect of the two groups was observed and compared. Results There was no significant difference in intraoperative blood loss and complication rate between the two groups (P>0.05); the operation time and postoperative bed rest time in the replacement group were significantly better than those in the fixation group, the differences were statistically significant (P<0.05). The total effective rate of the replacement group was 92.31%, and the total effective rate of the fixed group was 79.49%, the total effective rate of the replacement group was significantly higher than that of the fixation group, the difference was statistically significant (P<0.05). Conclusion The artificial joint replacement has remarkable clinical effect in the treatment of senile osteoporosis unstable intertrochanteric fractures, which is worthy of clinical promotion.
