两种切口白内障囊外摘除联合人工晶体植入术临床效果比较

摘要：

目的 比较两种切口白内障囊外摘除联合人工晶体植入术的临床效果。方法 回顾性分析本院2010年4月至2014年12月收治的白内障患者154例（175眼）的临床资料，按术式分A组（88眼），采用巩膜隧道切口白内障囊外摘除人工晶状体植入术；B组（87眼），采用透明角膜切口白内障囊外摘除人工晶状体植入术，对两组患者手术前、术后1周的裸眼视力、散光情况，手术时间，并发症情况进行分析与比较。结果 术后1周，B 组在0.6~1.0裸眼视力的患者多于A 组（P<0.05）；两组患者 的角膜散光度均降低，且B组角膜散光度明显低于A组（P<0.05）；A组手术时间<10 min 的患者占比低于B组（P<0.05）；两组患者术后的并发症发生率比较，差异不显著（P>0.05）。结论 透明角膜切口比巩膜隧道切口白内障囊外摘除联合人工晶体植入术具有简便、易行、经济，安全的优势，更适合于基层医院开展。

Effects comparison between the two modes of incision of extracapsular cataract extraction

and intraocular lens implantation

ABSTRACT: Objective To compare clinical effect between the two modes of incision of extracapsular cataract extraction and intraocular lens implantation. Methods The clinical data of 154 cases (175 eyes) of patients with cataract treated in our hospital from April 2010 to December 2014 were analyzed retrospectively. According to different operation methods, all the patients were divided into group A (88 eyes) underwent scleral tunnel incision, extracapsular cataract extraction and intraocular lens implantation and group B (87 eyes) underwent clear corneal incision, extracapsular cataract extraction and intraocular lens implantation. The uncorrected visual acuity, corneal astigmatism, operation time and complications were analyzed and compared between the two groups before and 1 week after operation. Results One week after operation, the number of patients with 0.6~1.0 uncorrected visual acuity in group B was more than that in group A (P<0.05). The corneal astigmatism were decreased in the two groups, and the corneal astigmatism in group B was lower than that in group A (P< 0.05). The ratio of operation time < 10 min in group A was lower than that in group B (P<0.05). There was no significant difference in postoperative complication rate between the two groups (P>0.05). Conclusion Compared with scleral tunnel incision, clear corneal incision, extracapsular cataract extraction and intraocular lens implantation is a simple, easy, economical and safe surgical method, which is more suitable for primary hospital.
