慢性乙型肝炎病毒感染者外周血细胞免疫功能的变化分析

摘要：

目的 观察慢性乙型肝炎病毒感染者外周血细胞免疫功能的变化及临床意义。方法 选择2013年1月至2016年10月我院收治的慢性乙型肝炎患者（乙肝组）100例、慢性乙型肝炎病毒感染者100例（携带组）及同期健康人员100例（健康组） 作为研究对象。抽取三组研究对象静脉血测定外周血中CD4+细胞、CD8+细胞、NK 细胞、DC细胞数量，测定外周血IL-17、 IL-1β、IL-6 水平，并进行比较。结果 三组外周血中CD4+细胞、CD8+细胞数量比较，差异无统计学意义（P>0.05）；与健康组比较，乙肝组及携带组NK 细胞数量减少、DC细胞数量增多，且乙肝组与携带组比较，差异均具有统计学意义（P<0.05）。乙肝组及携带组IL-17、IL-1β、IL-6 水平均高于健康组患者，且乙肝组IL-1β、IL-6 水平高于携带组（P<0.05）。结论 外周血NK 细 胞和DC细胞数量的检测可以反映慢性乙型肝炎病毒感染者的免疫功能的情况，且IL-17、IL-1β、IL-6 水平的变化在慢性乙型肝炎病毒损伤肝脏机制中发挥着重要作用。

Change analysis of peripheral blood cell immune function in patients with chronic hepatitis B virus infection

ABSTRACT: Objective To observe the change and clinical significance of peripheral blood cell immune function in patients with chronic hepatitis B virus infection. Methods From January 2013 to October 2016, 100 cases of patients with chronic hepatitis B (hepatitis B group), 100 cases of patients with chronic hepatitis B virus infection (carry group) and 100 cases of health people at the same period (healthy group) in our hospital were selected. The venous blood of all study objects were taken to detect the amounts of CD4+ cells, CD8+ cells, NK cells and DC cells in peripheral blood and the levels of IL-17, IL-1β, IL-6 in peripheral blood. Results There were no significant differences in the amounts of CD4+ cells and CD8+ cells in peripheral bloods between three groups (P>0.05); compared with the healthy group, the amounts of NK cells reduced, and amounts of DC cells increased in the hepatitis B group and the carry group, there were significant differences in amounts of NK cells and DC cells between the hepatitis B group and the carry group (P<0.05). The levels of IL-17,IL-1β, IL-6 in the hepatitis B group and the carry group were higher than those of the healthy group, and the levels of IL-1β, IL-6 in the hepatitis B group were higher than those in the carry group (P<0.05). Conclusion The detection of amounts of NK cells and DC cells in peripheral blood can reflect the immune function of patients with chronic hepatitis B virus infection, and the levels change of IL-17, IL-1β, IL-6 play an important role in the liver damage mechanism of chronic hepatitis B virus.
