内镜下高频电切与微波治疗胃息肉186例临床分析

摘要：

目的 观察内镜下高频电切及微波治疗胃息肉的临床效果。方法 186例胃息肉患者根据息肉类型、大小分别采用内镜下高频电切与微波治疗。观察胃息肉发生部位、大小及病理类型，观察治疗结果及随访并发症、复发情况。结果 186例患者胃息肉中，发生在贲门9例（4.84%），胃底98例（52.69%），胃体43例（23.12%），胃窦30例（16.13%），幽门6 例（3.22%），其中，胃底息肉发生率最高，其次为胃体息肉和胃窦息肉；病理类型：腺瘤息肉62例（33.33%），炎性息肉58例（31.18%），增生性息肉43例（23.12%），胃底腺息肉23 例（12.37%），其中腺瘤息肉和炎性息肉的发病率较高；内镜下高频电切与微波治疗手术成功率100.00%。186例患者随访期间出现出血3例，发生率为1.67%；远例患者出现不同程度的上腹部疼痛。结论 根据息肉大 小、性状、蒂解剖结构等选择内镜下高频电切、微波、内镜下黏膜切除术与内镜下黏膜剥离术治疗，临床效果显著，安全性好，值得临床推广应用。

Clinical analysis of 186 cases of gastric polyps treated by high frequency electric resection and microwave

irradiation in endoscope

ABSTRACT: Objective To observe the clinical effect of gastric polyps treated by high frequency electric resection and microwave irradiation in endoscope. Methods One hundred and eighty -six cases of gastric polyps were treated by high frequency electric resection and microwave irradiation in endoscope according to the type and size of gastric polyps. The location, size and pathological type of gastric polyps, and the treatment results, follow-up complications and recurrence were observed. Results In the 186 cases of patients with gastric polyps, there were 9 cases (4.84%) of gastric cardia, 98 cases (52.69%) of gastric fundus, 43 cases (23.12%) of gastric body, 30 cases (16.13%) of gastric antrum, 6 cases (3.23%) of pylorus. The incidence of gastric fundus had the highest incidence, and then the gastric body, gastric antrum. Pathological types: 62 cases (33.33%) of adenomatous polyps, 58 cases (31.18%) of inflammatory polyps, 43 cases (23.12%) of proliferative polyps and 23 cases (12.37%) of gastric fundus polyps, the adenomatous polugps and inframmatory polyps had higher incidence. The success rate of high frequency electric resection and microwave irradiation in endoscope were 100.00%. During the follow-up period, there were 3 cases bleeding, and the incidence was 1.67%, and there were 6 cases of patients with upper abdominal pain in different degree. Conclusion According to the size and character of the polyp and the anatomical structure, the effect of gastric polyps treated by high frequency electric resection and microwave irradiation in endoscope is remarkable and has good safety, which is worthy of clinical application.
