双切口双钢板内固定治疗复杂胫骨平台骨折的临床研究

摘要：

目的 探究双切口双钢板内固定治疗复杂胫骨平台骨折的临床疗效。方法 选取80例复杂胫骨平台骨折患者，按手术方法不同分为研究组与对照组，每组40例。两组患者分别实施双切口双钢板内固定治疗与锁定钢板内固定治疗，观察随访1 年后两组患者的临床疗效。结果 两组患者的手术情况及术后恢复时间比较无明显差异（P>0.05）。两组患者术后即刻、术后 3个月、1 年的TPA与PA度数比较无明显差异（P>0.05）。两组患者的HSS评分与膝关节恢复优良率比较无明显差异 （P>0.05）。对照组患者的并发症发生率明显高于研究组（P<0.01）。结论 复杂胫骨平台骨折选择双切口双钢板内固定治疗，与锁定钢板内固定治疗疗效相近，但并症发生率较低，可明显改善患者的生活质量，值得临床应用推广。

Clinical study of dual-incision double-plate fixation in the treatment of complex tibial plateau fracture

ABSTRACT: Objective To study the clinical effect of dual -incision double-plate fixation in the treatment of complex tibial plateau fractures. Methods Eighty cases of patients with complex tibial plateau fracture were divided into study group and control group, according to the different operation methods, with 40 cases in each group. They were treated with dual-incision double-plate fixation and locking plate internal fixation, respectively. After one year follow up, the clinical efficacy of the two groups was evaluated. Results There were no significant differences between the two groups in operation situation and postoperative recovery time (P>0.05). There were no significant differences between the two groups in the degree of TPA and PA (immediate postoperative, 3 months and 1 year after operation) (P>0.05). There were no significant differences between the two groups in the score of HSS and the excellent rate of knee joint recovery (P>0.05). The incidence rate of the complications in the control group is significantly higher than that of the study group (P<0.01). Conclusion The treatment of the complex tibial plateau fracture with dual-incision double-plate fixation has the similar clinical efficacy to locking plate internal fixation. But dual-incision double-plate fixation in the treatment of complex tibial plateau fracture has lower complications, which can improve the quality of patients` life, and deserve to be widely used.
