不同方式蓝光照射治疗新生儿黄疸的疗效与安全性分析

摘要:

目的 探讨间歇蓝光照射与持续蓝光照射治疗新生儿黄疸的疗效与安全性。方法 选取我院2014 年6 月至2016 年6 月收治的新生儿黄疸患儿80 例,按照随机数字表法分为间断治疗组和持续治疗组,间断治疗组2 次/d 进行蓝光治疗6~8 h 双面光疗,持续治疗组给予持续蓝光治疗16~18 h 双面光疗,观察两组患儿的治疗效果与不良反应发生情况。结果 治疗后两组患儿的胆红素值均明显下降(P<0.05),间断治疗组治疗24 h 后的胆红素值为(253.7±31.2)μmol/L,治疗72 h 后为(174.3± 22.2)μmol/L;持续治疗组治疗24 h后的胆红素值为(254.2±32.1)μmol/L,治疗72 h 后为(173.5±21.6)μmol/L,组间比较差异 无统计学意义(P>0.05);间断治疗组患儿的发热、腹泻、皮疹等不良反应发生率为12.5%,低于持续治疗组的32.5%(P<0.05)。结论 蓝光治疗新生儿黄疸疗效显著,持续、间断照射均可降低血清胆红素值,但间断治疗新生儿黄疸不良反应少,更安全,值得临床推广应用。

    Efficacy and safety of different ways of blue light in the treatment of neonatal jaundice with

    ABSTRACT: Objective To explore the efficacy and safety of intermittent blue light irradiation and continuous blue light irradiation in the treatment of neonatal jaundice. Methods Eighty cases of children with neonatal jaundice treated in our hospital from June 2014 to June 2016 were divided into intermittent treatment group and continuous treatment group according to random number table method. The intermittent treatment group was given twice blue light for 6 -8 hours double-sided phototherapy daily treatment, while the continuous treatment group was given continuous blue light treatment for 16-18 hours double-sided phototherapy. The therapeutic effects and adverse reactions in the two groups were observed. Results The bilirubin values of the two groups were significantly decreased (P<0.05), the bilirubin after 24 hours treatment of the intermittent treatment group was (253.7±31.2) mol/L, after 72 hours was (174.3±22.2) mol/L; the bilirubin value after 24 hours treatment of the continuous treatment group was (254.2±32.1) mol/L, after 72 hours was (173.5±21.6) mol/L, the diferences between the two groups were not statistically significant (P>0.05); the fever, diarrhea and rash incidence rate of intermittent treatment group was 12.5%, which was significantly lower than 32.5% of the continuous treatment group (P<0.05). Conclusion For neonatal jaundice, blue light has significant effect, both of continuous and intermittent irradiation can reduce the value of serum bilirubin, but intermittent has less adverse reactions with high safety, which is worthy of clinical application.
