瘢痕子宫再次妊娠行阴道分娩的临床效果及安全性分析

摘要：
目的 分析瘢痕子宫再次妊娠行阴道分娩的临床效果及安全性。方法 将300例瘢痕子宫再次妊娠产妇分为剖宫产分 娩组（n=163）与阴道分娩组（n=137），比较两组的分娩结局及住院时间。结果 阴道分娩组在新生儿Apger评分及新生儿窒息 率与剖宫产分娩组比较，无明显差异（P>0.05）；阴道分娩组较剖宫产分娩组产后出血量明显减少，新生儿体质量明显降低，住院时间明显缩短，差异有统计学意义（P<0.05）。结论 在满足阴道试产适应证的前提下，瘢痕子宫再次妊娠产妇最好行阴道分娩，但并非无风险，应做好产时监护帮助产妇顺利度过围产期。

Clinical effect and safety analysis of vaginal delivery of uterine scar pregnancy

ABSTRACT: Objective To analyze the clinical effect and safety of vaginal delivery in pregnant women with scar uterus. Methods Three hundred cases of uterine scar repregnant women were divided into cesarean delivery group (n=163) and vaginal trial production delivery group (n=137). The delivery outcomes and length of stay in the two groups were compared. Results Vaginal delivery group in the Apger score and the rate of neonatal asphyxia were not different from cesarean delivery group (P>0.05). In the vaginal delivery group, the amount of postpartum hemorrhage significantly reduced, the quality of newborn body significantly decreased, the length of hospital stay was significantly shorter, and the differences were statistical significance (P<0.05). Conclusion In the premise of meeting the indications of vaginal delivery, vaginal delivery is the best choice for uterine scar pregnancy maternal, but the method still has risk and the doctor should do intrapartum care to help women go through perinatal.
