电视胸腔镜下同期手术与分期手术治疗双侧肺大泡效果对比

摘要：

目的 对比电视胸腔镜在同期手术和分期手术在双侧肺大泡手术中的临床疗效。方法 回顾性分析2008年1月至2015年9月在本院胸外科进行双侧肺大泡切除的156 例患者的临床资料，按照不同手术方法分为试验组和对照组，各78例。试验组患者采取电视胸腔镜下行双侧同期肺大泡手术，对照组患者选择双侧肺大泡分期手术。对比两组患者术中、术后指标和术后并发症发生情况。结果 试验组手术用时（109.21±18.72）min、术中出血量（67.00±16.74）mL、手术切口长度为（1.81±0.84）cm，均明显优于对照组的（137.40±23.52）min、（141.00±29.83）mL、（20.33±5.65）cm；术后试验组患者的心率、疼痛时间、 止痛用药、首次下床活动时间和住院时间均优于对照组；且试验组术后并发症总发生率为7.69%，明显低于对照组的21.79%（P<0.05）。结论 在电视胸腔镜下进行同期双侧肺大泡切除手术，较行双侧肺大泡分期手术的临床疗效更好，具有风险小、创伤小、恢复快等特点，适合临床推广应用。

Comparison of video-assisted thoracoscopic surgery and staging surgery in the treatment of bilateral pulmonary bullae

ABSTRACT: Objective To compare the clinical efficacy of video-assisted thoracoscopic surgery and staging surgery in the treatment of bilateral pulmonary bullae. Methods The data of 156 cases receiving bilateral pulmonary bullous resection from January 2008 to September 2015 in thoracic surgery of our hospital were retrospectively analyzed, the patients were divided into experimental group and control group according to different treatment methods, with 78 cases in each group. The experimental group were treated with video-assisted thoracoscopic surgery, and the control group were treated with staging surgery. The intraoperative and postoperative indexes and complications were compared between the two groups. Results In the experimental group, the operative time was (109.21±18.72) min, bleeding loss during operation was (67.00±16.74) mL, the incision length was (1.81±0.84) cm, which were significantly better than (137.40±23.52) min, (141.00±29.83) mL, (20.33±5.65) cm in the control group (P<0.05); the heart rate, duration of pain, analgesic medication, first time of getting out of bed and hospital stay were superior to those in the control group (P<0.05); the total incidence rate of postoperative complication was 7.69% , which was lower than 21.79% in the control group (P<0.05). Conclusion The clinical curative effect of video-assisted thoracoscopic surgery in the treatment of bilateral bullae is remarkable, with less risk, small trauma, rapid recovery, which is worthy of clinical popularization.
