冠脉CTA在冠心病临床诊断中的应用

摘要:

目的 对比分析冠脉CTA与冠脉造影诊断冠心病的临床特点,为冠脉CTA的应用提供参考。方法 采用前瞻性研究方法,选择2014年2月至2015年2月我院心内科因胸痛、胸闷等症状临床确诊或疑诊为冠心病120例住院患者,均进行冠脉CTA与冠脉造影诊断,对冠状动脉斑块与狭窄情况进行记录与分析。结果 120例患者中,冠状动脉造影判断为冠心病患者98例(冠心病组)和非冠心病患者(非冠心病组)22例,两组患者的性别、年龄、体重指数、甘油三酯、血糖差异不显著(P>0.05);冠心病组的冠状动脉斑块多为软斑块,非冠心病组多为硬斑块,两组差异有统计学意义(P<0.05);冠心病组多为中度狭窄与重度狭窄,而非冠心病组多为正常与轻度狭窄,两组差异显著(P<0.05)。结论 冠脉CTA诊断冠心病具有较好的效果,能很好地反映斑块的性质与冠状动脉狭窄情况。

    Application of coronary CTA in clinical diagnosis of coronary heart disease

    ABSTRACT: Objective To discuss the clinical chracters of coronary CT (CTA) and coronary angiography (CAG) in diagnosis of coronary heart disease, and to provide the references for application of the coronary CTA. Methods Prospective method was used in this study. A total of 120 cases of patients with coronary heart disease diagnosed with chest tightness, chest pain and other clinical symptoms were admitted in our hospital form February 2014 to February 2015. All the patients were diagnosed or suspected as the coronary heart disease, and accepted CTA and CAG detection, the plaque and stenosis of coronary were recorded and analyzed. Results Among 120 patients, 98 cases diagnosed as coronary with CAG were classified into coronary group, while 22 cases not diagnosed as coronary heart disease were classified into non -coronary group. There was no significant difference on sexual, age, diabetes mellitus history, body mass index (BMI), triglyceride and blood glucose between the two groups (P>0.05); coronary plaque in coronary group was soft plaque, while hard plaque in non -coronary group, and there was significant difference between the two groups (P <0.05). While intermediate stenosis and severe stenosis were shown in coronary group and the light stenosis and no stenosis in non -coronary group, the difference was significant between the two groups (P<0.05). Conclusion Coronary CTA has better efficacy in diagnosis of coronary, it can reflect characters of plaque and the stenosis of coronary.
