氟喹诺酮类药物不良反应及合理用药分析

摘要:

目的 探讨氟喹诺酮类药物不良反应(ADR)及其合理用药。方法 回顾性分析我院2015年7月至2016年7月收治的90例应用氟喹诺酮类药物治疗并产生不良反应的患者的临床资料,综合患者的性别、年龄、给药方式和途径、不良反应发生时间及其临床表现等进行分析,探讨氟喹诺酮类药物的临床合理用药情况。结果 出现氟喹诺酮类药物不良反应的患者男性较女性多;年龄在31~40岁阶段的患者ADR发生率最高(46.67%)。氟喹诺酮类药物单独给药的ADR发生率(75.56%)高于联合用药(24.44%);静脉注射的ADR发生率(63.34%)明显高于口服用药(23.33%)和外用用药(13.33%)。氟喹诺酮类药物ADR在用药后10~30 min 和1~24 h的发生率最高;恢复时间在用药后1~24 h比例最高(41.11%)。氟喹诺酮类药物应用所 致的ADR可累及人体多个器官和系统,且其临床表现具有多样性;90例应用氟喹诺酮类药物出现ADR的患者,治愈率为 32.22%、好转率64.44%、无1例死亡,转归情况良好。结论 在氟喹诺酮类药物的临床应用中,医师应严格掌握其适应证和禁 忌证,并根据患者病情选择合适的给药途径和方式,避免不合理的联合用药,加强用药过程中患者ADR的监测,才能有效降低氟喹诺酮类药物的ADR 发生率。

    Analysis of adverse drug reactions and rational drug use of fluoroquinolones

    ABSTRACT: Objective To study the adverse drug reactions (ADR) and rational drug use of fluoroquinolones. Methods The clinical data of 90 patients treated with fluoroquinolone and occurred ADR in our hospital from July 2015 to July 2016 were retrospectively analyzed. In consideration of patients` gender, age, approach and ways of drug administration, incidence time of adverse effect and clinical features were statistically analyzed and the rational drug usage was discussed. Results The number of male appeared ADR after performing fluoroquinolones was higher than that of female, and the ADR was frequently appeared in patients with age of 30 -40 years (46.67% ). The incidence rate of ADR of performing fluoroquinolones alone (75.56%) was more than that of drug combination (24.44%). The incidence rate of ADR of venous administration (63.34%) was higher than that of orally taking medicine (13.33%) and externally using medicine (13.33%). The incidence of ADR of fluoroquinolones within 10-30 minutes and 1-24 hours after taking medicine were the highest than other periods; and the recovery was best in 1-24 hours after taking medicine. The ADR of fluoroquinolones involved multiple organs and systems, and the clinical features showed diversity. Among 90 cases, the cure rate was 32.22% , percentage of improvement was 64.44%, and recovery was better without death. Conclusion During the clinical application of fluoroquinolones, physicians should strictly grasp the indications and contraindications, to chose the appropriate approach and ways of taking medicine according to patients condition, and avoid the adverse reations of medicine. Hence strengthening the monitoring during medicine administration can reduce the incidence of ADR.
